Hugh Calkins signed this procedure report, stating two sites had been ablated, then later wrote the addendum under his signature, reporting that “We proceeded to isolate the RSVP successfully.” It seems that “We” in this case did not include him, and that the attending did not perform the procedure as he stated for the record.
It appears by what he told family members that he was initially unaware that someone had restarted the ablation process after Pam’s mitral valve was destroyed.
